It's relatively easy to change your Health Maintenance Organization (HMO), but you there may be restrictions on when you may do it. If you receive health insurance through your employer, there is typically an open enrollment period once a year when you are allowed to switch HMOs. You can also usually change your HMO if you move. Follow the instructions of the HMO provider to be sure that you have coverage at all times.
Instructions
1. Talk to your human resources department at your place of employment about your options for changing your insurance plan. They can help you review your options and give you any forms that you may need.
2. Contact your new HMO. Ask about the enrollment process and obtain the necessary forms. You may be able to download the forms from the company website, or you can have the company mail them.
3. Submit your application to the new HMO. If you're applying for individual insurance, you may need to submit supporting documents, such as proof of residence, proof of employment and proof of income.
4. Receive the new insurance cards and make a payment. This means that your new insurance has started. It will usually begin on the first of the month.
5. Tell your old HMO that you will be changing providers. This will cancel your health insurance through this provider.
