Fill Out A Health History Form

Whether you're applying for a certain job, seeing a new doctor, getting insurance or even buying personal training sessions, you probably will be asked to fill out a health history form. In today's information-based society, these forms give potential employers and health-care providers a glimpse into your personal health history. The rule to remember when filling one out is to be honest.


Instructions








1. Fill out your name and date at the top of the form. Take your time and write legibly so that the form can be both read and understood by the appropriate personnel. Fill out the section that gives your Social Security number, current residence and other basic information.








2. Fill out the next section or related blanks pertaining to age, height and weight. In some cases, activity level may be asked (sedentary or moderately or very active), as well as information relating to any smoking, alcohol use or recreational or habitual drug use. Again, be honest. If you're in a doctor's office, fill out the symptoms questionnaire.


3. Think over your health history for the next section(s). These will typically ask for information regarding current or previous prescription medications, as well as any surgeries. The surgeries section will ask for dates, and if you can't remember, give an approximate date. Finish the form by signing (and dating, if asked). Recheck the form to ensure that you have filled out the correct boxes, and give it to the receptionist, nurse or related personnel.
