Instructions
1. Contact the Social Security Administration to make an appointment to file a disability claim for your child (see Resources below).
2. Gather the necessary information and documents for the interview. You will need proof of income, such as bank statements, check stubs and tax records. You will also need proof of any resources you might have, such as cars or homes that you own. You will also need to gather documents pertaining to your child's medical problems. You will need to know the name, address and phone number of your child's primary care physician, as well as information about how your child is doing in school, such as report cards and letters from their teacher.
3. Obtain a certified copy of your child's birth certificate by visiting the Department of Health in the city where your child was born. You will need to apply for a certified copy of the birth certificate and you will receive it several days later by mail.
